As a health professional you have seen first hand the disability, disease and death caused by smoking among your patients. As a person who smokes, you also understand the addictive nature of nicotine.
You can use this professional and personal knowledge to inform and empathize with your patients. Remember that you also have access to a wide range of professional resources to help others (and yourself) to take action.
As a health professional you may need to make decisions about your own private reasons for continuing to smoke. Even though your choice may be to continue to smoke, you may want to consider changing some of your usual smoking habits while at work.
