THE ASK, ADVISE, ASSIST, ARRANGE (4A'S) PROTOCOL

Below is the 4As Protocol for a Smoking Cessation Intervention.  It is recommended that you implement this minimal contact intervention (lasting 1 to 3 minutes) with all clients.  Every health professional should:

ASK: about tobacco use at every contact with all clients.

ADVISE:
all tobacco users to consider quitting.

ASSIST:
 by providing minimal contact intervention, or according to client readiness/stages of change.

ARRANGE: 
referral and follow-up.

4 a's

NOTE:
  Although this protocol refers to “As your nurse…” the approach is applicable for all health professionals.

As a method of documenting this exchange with your patients, we suggest that you consider creating a sticker for your patient charts that identifies those who smoke and what cessation support were provided at each interaction.
