If you are pregnant and would like to book an appointment to receive the unadjuvanted H1N1 vaccine, please call 743-1000.

The Health Unit is running a clinic for children aged 6 months to 9 years who require a second dose of the adjuvanted H1N1 vaccine on Saturday, January 9, 2010 from 10 a.m. to 2 p.m.  If you would like to book an appointment at this clinic for your child, please call 743-1000

H1N1 Vaccine Consent Form*

*If you are bringing someone else’s child to get vaccinated (17 years and under), please print out and complete the consent form and ensure it is signed by the child’s parent or guardian BEFORE visiting the clinic.  Children who do not have consent forms signed by their parent of guardian will not be vaccinated.

Adults 18 years and older will complete this form electronically when they arrive at the clinic, so please do not fill it in ahead of time.
